Having a Primary Care appointment is great to have whether you are feeling healthy or sick. Everyone needs to have a primary care provider that has the "baseline" history and status of health and wellness. Here are a list of things you can expect or inquire about at a Primary Care Visit:

1. ANNUAL WELLNESS EXAM

Vitals, physical exam with possible EKG, blood work lab order to be completed at Quest, LabCorp or another facility accepting insurance or self pay.

2. SCREENINGS

Mammogram

Chest Xray order for lung cancer screening.

Bone Density Order for osteoporosis screening.

STI/STD blood and urine screening.

Screening for anemia, electrolyte imbalance, kidney function, liver function, diabetes, thyroid issues, urinary tract infections, cholesterol screening, and more.

3. MANAGEMENT

Such as: Diabetes, hypertension, mental health, birth control, hypothyroidism, and more.

**Step 1:** Schedule an initial visit to take a medical history and perform a physical examination. During this visit, we will also arrange for blood work to be completed either through an insurance-approved lab or on a self-pay basis.

**Step 2:** Review the lab results in a follow-up appointment, which can be conducted via telehealth or in person. Prior Authorizations for medication will be sent as necessary for certain insurance plans. Medication options include: Phentermine, Semaglutide (Ozempic, Wegovy), Tirzepatide (Zepbound), B12 injections (Cyanocobalamin), and more!

**Step 3:** Based on the results, treatment may include biannual blood work. Weight management program requires monthly visits with virtual and in person options.

**Step 3:** Based on the results, treatment options may include testosterone injections, nasal testosterone, or topical testosterone cream. If testosterone treatment is not suitable, there are alternative options such as Cialis, Viagra, penile injections, or penile implants. Additionally, we will provide anti-estrogen medication, as well as the necessary needles, syringes, and any other supplies, on a monthly basis.

Rapid urine drug testing is available at the office if needed. Suboxone prescription is given on the first visit.

**Step 2:** Review the lab results in a follow-up appointment, which can be conducted via telehealth or in person.

**Step 3:** Based on the results, the patient will be in the Suboxone medicated assistance program on a monthly basis. This program requires signing the Focused Wellness Clinic agreement for treatment and compliance, urine drug testing, and monthly visits. We offer affordable self pay prices and some insurances are accepted.

** In the state of Florida, all drivers applying for a CDL must have a DOT medical card for non-excepted interstate and non-excepted intrastate travel. DOT physicals are required before you receive your CDL license. They must be filed as part of your CDL application. Focused Wellness Clinic (FWC) has a qualified medical examiner to administer DOT physical examinations, APRN Constance is listed in the Federal Motor Carrier Safety Administration National Registry.

**Step 1:** Book an appointment. If you have medical concerns please list those concerns in the booking "comments" or "additional" section. We can provide a medical release form to you to have your records sent prior to your appointment. Our goal is to get you certified and on the road safely.

**Step 2:** The general exam would likely include looking at your vital signs, overall appearance, listening to your heart, lungs, bowels, examining your spine, ears, throat, mouth, etc. Testing your reflexes, strength, grasp, mobility, nervous system. Drug urine testing, blood glucose, and protein levels in urine.

**Step 3.** Upon completion of a passing exam, you will receive a medical certification good for two years. A copy of the medical certification will also be electronically transmitted to the Department of Transportation.

Things that may be needed (FWC can provide a medical release form that you sign and we fax to previous providers or specialists for you):

A medication list, doses, and medical provider who prescribed it. Also pharmacy who filled it.

The name of the office and medical Optometrist for your eyeglasses, contacts, or hearing aids if you use them.

Test results of your last blood work for diabetes and a log of your blood glucose if you are a person with diabetes

CPAP readings and name/office of Pulmonologist if you are a person with Sleep Apnea

Visit summaries and or letter of clearance from your Cardiologist and results from your most recent stress test and ECHO cardiogram if you are a person with a heart condition

Any other pertinent history.

Focused Wellness Clinic (FWC) can assist in non-emergent cases of:

Cold-like symptoms (cough, sore throat, etc), Covid or Flu testing and treatment,

Sinus infection, urinary tract infection (UTI), Shingles, shortness of breath on

exertion, STI/STD testing and treatment and more.

**Price of $55 is reflecting base self-pay visit, not including price of medication. If you have an insurance plan accepted by Focused Wellness Clinic, the price of visit will be based on the insurance contract with the Provider. Any in office test with same day results incurs an additional $20 charge. Any in office test collected and sent off for testing incurs an additional $10 charge.
