Having a Primary Care appointment is great to have whether you are feeling healthy or sick. Everyone needs to have a primary care provider that has the "baseline" history and status of health and wellness. Here are a list of things you can expect or inquire about at a Primary Care Visit:

1. ANNUAL WELLNESS EXAM

Vitals, physical exam with possible EKG, blood work lab order to be completed at Quest, LabCorp or another facility accepting insurance or self pay.

2. SCREENINGS

Mammogram

Chest Xray order for lung cancer screening.

Bone Density Order for osteoporosis screening.

STI/STD blood and urine screening.

Screening for anemia, electrolyte imbalance, kidney function, liver function, diabetes, thyroid issues, urinary tract infections, cholesterol screening, and more.

3. MANAGEMENT

Such as: Diabetes, hypertension, mental health, birth control, hypothyroidism, and more.

**Step 1:** Schedule an initial visit to take a medical history and perform a physical examination. During this visit, we will also arrange for blood work to be completed either through an insurance-approved lab or on a self-pay basis.

**Step 2:** Review the lab results in a follow-up appointment, which can be conducted via telehealth or in person. Prior Authorizations for medication will be sent as necessary for certain insurance plans. Medication options include: Phentermine, Semaglutide (Ozempic, Wegovy), Tirzepatide (Zepbound), B12 injections (Cyanocobalamin), and more!

**Step 3:** Based on the results, treatment may include biannual blood work. Weight management program requires monthly visits with virtual and in person options.

**Step 3:** Based on the results, treatment options may include testosterone injections, nasal testosterone, or topical testosterone cream. If testosterone treatment is not suitable, there are alternative options such as Cialis, Viagra, penile injections, or penile implants. Additionally, we will provide anti-estrogen medication, as well as the necessary needles, syringes, and any other supplies, on a monthly basis.

Rapid urine drug testing is available at the office if needed. Suboxone prescription is given on the first visit.

**Step 2:** Review the lab results in a follow-up appointment, which can be conducted via telehealth or in person.

**Step 3:** Based on the results, the patient will be in the Suboxone medicated assistance program on a monthly basis. This program requires signing the Focused Wellness Clinic agreement for treatment and compliance, urine drug testing, and monthly visits. We offer affordable self pay prices and some insurances are accepted.

We are here for you if you are feeling sick. However if it is a medical emergency, please call 911, report to ER or Urgent Care.

Focused Wellness Clinic (FWC) can assist in non-emergent cases of:

Cold-like symptoms (cough, sore throat, etc), Covid or Flu testing and treatment,

Sinus infection, urinary tract infection (UTI), Shingles, shortness of breath on

exertion, STI/STD testing and treatment and more.

**Price of $55 is reflecting base self-pay visit, not including price of medication. If you have an insurance plan accepted by Focused Wellness Clinic, the price of visit will be based on the insurance contract with the Provider. Any in office test with same day results incurs an additional $20 charge. Any in office test collected and sent off for testing incurs an additional $10 charge.
